As a service to both current students and alumni, the Examinations section of the University has provided a number of forms that can be downloaded and submitted for specific purposes.

Academic Bank of Credits (ABC)
In line with the National Education Policy 2020, the Ministry of Human Resources Development (MHRD), Govt. of India, has mandated that all students in India have a unique 12-digit ID that remains with them through their academic journey from school to university and beyond. The National Academic Depository (NAD) was born out of this initiative.
The 12-digit ID is called the Academic Bank of Credits (ABC). It has several benefits for students, including a repository and authentication of their degrees, diplomas, certificates, training details, and co-curricular accomplishments year after year. It also allows students to participate in the credit transfer system between institutions.
Students can get their ID by visiting the official website of Academic Bank of Credits (ABC).
Note: The ID can also be obtained from the Digilocker NAD website. Furthermore, school students may already have their ID from the Automated Permanent Academic Account Registry (APAAR) portal. All these 12-digit IDs are the same, i.e., part of the National Academic Depository (NAD).
All SSSIHL students have obtained their 12-digit ID and are part of the NAD.

Application for End Semester Supplementary Examination Registration
This is the Application to register for the Supplementary Examinations, to be submitted by the candidate through the Director of her/his Campus of study.
Application for Re-Totalling of Marks
This is the Application to seek re-totalling of marks in a paper in the End Semester Examination, and is to be submitted directly to the office of the Controller of Examinations, along with necessary enclosures as instructed, within 10 days from the date of receipt of the respective Semester Grade Cards by the campus office.
Application for Duplicate Grade Card
This is the Application to seek a Duplicate Grade Card and can be used only by those candidates who have lost their original Grade Cards. This form is to be submitted directly to the office of the Controller of Examinations, along with necessary enclosures as instructed.
Application for Duplicate Degree Certificate
This is the Application to seek a Duplicate Degree Certificate and can be used only by those candidates who have lost their original Degree Certificate. This form is to be submitted by the candidate through the Director of her/his Campus of study, along with necessary enclosures as instructed.
Application for Migration Certificate
This is the Application to seek a Migration Certificate and is to be submitted by the candidate through the Director of her/his Campus of study, along with necessary enclosures as instructed.
Application for Change of Name
This is the Application to register the changed name of a candidate in the Institute records and is to be submitted by the candidate through the Director of her/his Campus of study, along with necessary enclosures as instructed.
Application for CGPA to Percentage Conversion
This is the Application to seek statement of conversion of Cumulative Grade Point Average (CGPA) into equivalent percentage of marks. The form is to be submitted by the candidate to the office of the Controller of Examinations, along with necessary enclosures as instructed. This form is applicable only for the candidates who have completed their programme of study from the Institute in March 2009 or before.
